A neck seam can be caused by mismatched textures (typically normal or specular maps) or improperly flagged body meshes. A neck gap can be caused by using an imported head that was exported for a different body weight, or using an outfit that only supports one extreme (i.e. 0% or 100%) weight due to not having a working weight slider.
