If you Google D1-4 spindle and look at images, there are a number of locations that show the dimensions of the nose. It is my understanding that the chuck should fit with about 0.001″ gap to the face of the lathe nose not like you describe.

After doing a little investigating, I believe the tapered nose part of the D1-4 mount is the problem. I don’t know if the spindle is too large or if the chuck is too small, but an interference is preventing the chuck back to pull up against the spindle face. So, what happens is when the cam-locks engage, they pull or tilt the chuck off axis.
